Therapeutic Foster Care: for children
with significant developmental, behavioral, and/or emotional support needs.
Service includes: point and level system/behavioral programming, and up to 4
hours of skills training per week. In addition to this, children and their
family are assigned a therapist at Family Pathways; Psychiatric Evaluation and
medication management are available; these services are billed through MA.
Supervised visitation between the parent and child is also coordinated by Family
Pathways.
Emergency Placement and Stabilization: short term (30 to 60 days) with assessment of treatment and support needs.
Psychosocial Support Foster Care: Specifically designated for young children with mild developmental, emotional, behavioral, and/or healthcare needs that do not meet the criterion for more extensive placement services.
Medically Fragile Foster Care:
Level I: Child’s medical condition can be controlled through medical supervision and consistent non-specialized care. (failure to thrive, drug addicted infants, broken limb, blind, deaf, non ambulatory, diabetic, medical needs related to abuse or neglect).
Level II: The child’s condition is not life threatening and can be controlled through a specialized intervention, medical supervision, and consistent care. (colostomy, cerebral palsy, medical or sensory conditions coupled with emotional problems, any condition requiring a medical apparatus).
Level III: Children who may be terminally ill or other debilitating condition that can be managed with some specialized extensive care.
Level IV: Children with acute chronic or permanent medical conditions that require intensive home based medical intervention.
Providing
foster parents trained in behavior modification strategies. Children also
receive psychiatric
services, individual and family therapy, a skills trainer, case management and
monthly reports. These reports include child development/improvements, concerns, recommendations and safety
contact status for each placement. Each child receives an individual Service Plan.
