Forever Family provides a host of Adoption and Permanency services designed to support children and parents as they strive to expand their family. This program is licensed by the PA Department of Public Welfare and an affiliate of the Statewide Adoption & Permanency Network.

Services:

•  Locate permanent resources for waiting children – Child Specific Recruitment
•  Preparation for children & families – Child & Family Preparation
•  Study of adoption applicants – Family Profile
•  Placement and Supervision of children in permanent settings – Placement & Finalization
•  Pre and Post Adoption Support – Post Permanency Services

Child Assessment (Profiles): A written document that contains a comprehensive overview of the child's medical, psychological, educational, and placement histories and significant information regarding the child's family background and special needs.

Matching: Activities designed to offer the child the best possible opportunity to find a foster or adoptive family, using waiting child albums, videos, and the media.

Family Profile and Home Study: A comprehensive written assessment of adoptive families strengths and abilities, resources, composition and lifestyle, and needs. The Family Profile is produced with each family's input, the information is reviewed by each family before its submission.

Child Preparation: A process designed to ensure that the child is prepared and ready for adoption. Staff will develop a written plan, and work with the child about grief, loss, and development of attachment.

Post Place Monitoring: Helps to ensure the positive adjustment of the family and the adopted child. Forever Family will provide regular and ongoing support throughout the post placement period. Services include home visits and telephone support; crisis intervention and respite, if needed. Staff will also assist with accessing specialized resources and provide additional training specifically designed to address the child and family's unique needs. There will also be networking opportunities to interact with other adoptive families and assistance will be provided in preparing the documents for the legalization of the child's adoption.

Post Adoption Services: Are designed to help families recognize their strengths, identify their needs and find support in their communities. Services include:
